I need a custom Google Form and linked Google Sheets system to track CRNA hours, calculate pay, and generate monthly summaries. This will be used by CRNAs and supervising physicians across multiple workdays. Requirements: ✅ Google Form that collects: Employee Name (dropdown from list) Date worked Start Time End Time Supervising physician (dropdown from list) Optional: hourly rate (if not fixed, default to $150/hour) ✅ Google Sheet that: Automatically calculates hours worked per day Multiplies hours × hourly rate to calculate pay Adds a “Month” column from the date Includes a dynamic summary table showing total hours and pay per employee per month Optional: a dashboard or visual report Optional: flag if end time is before start time Optional: restrict form to known emails or add data validation
