Google Apps Script Developer – Automate PDF Reports from Google Forms for Mental Health Practice

We are a private psychology practice specializing in anxiety and OCD. We currently use Google Forms to collect outcome measures (e.g., for OCD, ARFID, BDD, etc.). The responses are automatically scored and stored in Google Sheets. However, we need to automate and refine the next steps: Scope of Work: We’re looking for someone to create a system that: Generates a PDF report from each Google Form submission using a custom Google Docs template. Names the PDF file with patient name or ID + date (e.g., Smith_John_OCD_2025-04-21.pdf). Saves the PDF to a specific folder in Google Drive based on the diagnosis (e.g., /Outcome Measures/OCD/Patient Reports/). Emails our front office staff with a link to download the PDF for upload into our EHR (TherapyNotes). Updates a Google Sheet tracker to show whether the PDF has been uploaded to our EHR system. Requirements: Proficiency with Google Apps Script Experience automating workflows with Forms, Sheets, Docs, Drive, and Gmail Ability to work with structured data and dynamic folder creation Comfortable creating maintainable, documented code (Preferred) Familiarity with HIPAA-compliant workflows (even if we are not storing PHI in the automation) Deliverables: Fully functional and tested Google Apps Script Easy-to-maintain documentation or walkthrough Folder logic that dynamically sorts PDFs by diagnosis Optional: Interface or checklist tracker for staff use Nice to Have: Experience working with private healthcare practices Ability to build a future internal dashboard or flag system (uploaded vs pending) Familiarity with TherapyNotes (not required, but helpful) To Apply: Please include: A brief summary of similar projects you’ve done A short explanation of how you’d approach this automation Whether you’re available for ongoing support if needed
