Dynamic VBA form object

1) Create an EXCEL VBA project that has macro/function/sub to create and destroy a VBA form with name and age fields and two button cancel and ok. 2) the function above should be executed via a button created in the Add-Ins menu. 3) it should not reset the project when calling function #1 to create and destroy the form. Create two global variables and init them in worksheet open, make sure these variables do not get reset to empty once we destroy and create the form again.
