Quote PDF Export from HTML Form

I need a HTML quote form where you can type client's details, choose products from a list, add to the quote, form calculate totals and at the bottom of the page a button to export it as PDF generates a PDF based on a template we had designed professionally with product name, price, description and thumbnail, PDF template has a cover page and a T&Cs page at the end.
