Live - Multi user intake form


I would like to have a "live" intake form created where multiple people can add to it, updating other team members as it is filled out. The final form (or while it is filled out) would be dumped into a central area such as a Goggle spreadsheet. I have already created one in Google Forms, but it lacks the multi user ability and email notification as updates are made by other users I am hoping for. There is also a possibility I will need a PDF form made editable for a different form our nurses use during assessments. This is still in a "to-be-determined" state. I am not sure if a Goggle Doc would be better for any of these. I am open to suggestions. I have attached the word doc that I used to create the Google Form. If you would like to see the actual Form, let me know how to share it with you.
