Trending Healthcare Data in MS Excel

Hi, this project is pretty straight forward in that I have some data for about 1-3 years' worth (PHI removed) that I would like to have trended. The data that I have is controlled substances discrepancies (un-resolved vs resolved) and I would like to show over time (month to month, versus year to year) on how well the discrepancies were resolved or not resolved. I would like to have the data show how well each month did better or worse and perhaps what team members were involved with creating more discrepancies and or having the fewest vs the most discrepancies resolved vs un-resolved. There is much I would like to have the data show/displayed in graphs or charts (in color) that can tell a story either way. Once this data is displayed then show me how it was created to re-create this going forward. I can provide better idea of what I am looking for as closer as I get to selecting the individual for this project since it is sensitive etc. The data is already in MS Excel and should be straight forward. I will also take recommendations on the best way to present the data so that it's easy to read and understand for the average viewer, so recommendations are very much welcomed!
