Step 3: Select Make (Query Database, return list of models populating drop down) Step 4: Select Model (Query Database, return list of Body Styles in drop down) Step 5: Select Body Style (Query Database, return list of Repair Types Available) Step 6: Select Repair Type Step 7, click next Step 8: Enter Personal Information (Name, Phone, Email) Step 9: Enter Insurance Information (4 fields, Do you have insurance? Insurance Company, Deductible, Comments.) Step 10: Click Next for confirmation screen to return all values Step 11: Submit (Submits information to CRM database) I think it might be best to write this as a WordPress plugin so it can be embedded on a page template via a shortcode.
