Vehicle intake form

I own an automobile repair shop and I need a Vehicle condition/intake form created. This is basically a quick and simple form for the tech to fill out when they go to bring the vehicle into th shop. I have attached a couple things. the actual inspection sheet I liked the look of the rounded text boxes flows better Thant he normal inspection reports auto repair. I attached a vehicle image image. I want an image similar included. it does not have to be that image specifically The written document is basically the flow and desired info im wanting but in a format like the provided inspection sheet. I want simple. Black and white for easy duplicating I need someone who can make a quick edit after I use the form for a week or two. Forinstace maybe I want the order of it changed or something omitted and added. I need it simple enough that it gets used every single tie in its entirety but comprehensive enough to get the info we need to go further. This is not an Inspection form. think f it as a preliminary check. this info tells us if we need to look further into something, and it tells the people at the desk the staff actually looked at the vehcile while it was in shop. IF you are able to take the pieces I have and make what im looking for please let me know if not please dont waste each others time. Please submit your estimated costs and anything you have done that is similar. I understand this isn't a huge Job this will likely spill over into more work as I develop workflows and flow charts for processes etc. Thanks
