Regional Carriers: After covering national carriers, add insurance companies that operate regionally and may be more relevant to users in specific states or areas. Quantity: Aim to provide a comprehensive list, ideally covering as many carriers as possible. The number is flexible, but we need a diverse range of options for different regions. - For each carrier, confirm: - Written confirmation of coverage for the wheelchair power assist device under a specific HCPCS code. - The specific eligibility criteria (e.g., prescription requirements, covered plans, medical conditions, age requirements, etc.). - Any regional or location-based restrictions, including cities, states, or zip codes where coverage may vary. - Plan restrictions (e.g., only certain types of plans or coverage tiers are eligible). - Provide any necessary contact information or links where the consumer or dealer can submit claims, prescriptions, other required information, and vendor details so the insurer knows who to expect the invoice from when processing the claim.
